In general your need to establish "medical necessity" for the treatment in order to have an insurance company pay for services. This is usually done in the form of a prescription from a Medical Doctor or Chiropractor, who is treating the client for a condition. Next the treatment itself has to be one that the insurance company covers. Thats the basic plan. SOAP notes usually need to be submitted with the claim, which must be on a HCFV form (pronounced HIC-VA). You can order these from any medical supply company, massage warehouse probably has them.
