Mobile Applications to help with your practice.

I have been looking for a mobile app for my Android device that will allow me to do my SOAP charting electronically. I once did an On-call massage and I completely forgot my SOAP chart so I brought my laptop. It was nice to be able to enter the data digitally but even a laptop is quite cumbersome when you are trying to assess someone. I would be great if there was a mobile app. i could put on my tablet. I've looked around online but I can't seem to find anything. Does anyone know of any App or program that will do this. Or even a program that I could create my own form that I could some how make interactive and make my tablet store it until I got to my comp. I would LOVE to reduce my paper consumption!
