Online Intake Form
Has anyone found and used an online based intake form that will also store the information for you, so that when you want to access the information you just have to type the persons name in and all of their information pops up. Also if you have found a program like this- have you found one that will also allow you to do soap notes after the fact? Please, any and all information would be greatly appreciated. I want to get away from paper, and have everything at the touch of the button so as to make it easier to look up clients when the come in. I was thinking about having this technology on a tablet of sorts. Thanks for any ideas and information!

I use SoapVault. It allows you to email the form to your client before hand and when your client fills it out the form is automatically saved on the internet. You can have your client fill out the form when they get in as well. You definitely can go back later to add soap notes, very thorough medical history form. It even has easy links to copy and paste to your website for online booking. SoapVault does offer a free trial period, I think it was 30 days to give it a try. So far I'm really satisfied with the program.

Anne Denton- Thank you for the information, I will be checking this out!! I was going to try to create my own form from jotform. I have some more checking to do on it. They say there is a way to send the information securely from jotform but I just want to check the both out and see what is the best option for me and my business. Thanks for the tip!

Wendy Anne and Ashley - have you made any progress / have any more feedback? We recently developed a massage trigger points iPhone app and are looking to develop more solutions for the profession. I'd love to understand what exactly your needs are, what is not so good about current solutions, what do you specifically want include/want the software to do. The more details I have from you the more we can customize.
