Billing NYS no-Fault insurance

DOes anyone know how to do this?  or a link to tell me how?  I know I need to do soap notes, I can find the form, and I know the CPT codes-- but where are the times, (how many 15 minute treatments can the patient receive per day? do I have to be on some kind of list? or just use my license number?)  thanks!
