What states allow you to bill HMO's and PPO's?

Billing insurance is so different in each state.  I know here in WA we are one of the only states that are actually allowed to be contracted providers with insurance companies.  Fl massage therapists can bill insurance but I don't think they are contracted. 

What other states can you get paid by an PPO or HMO (Blue Cross/Blue Shield and all of the others)?

I would like to create a list of them and also have a resource for learning to bill in each state to find out how to do it from every step of the way.  Provide as much info as you can.

Can you bill a PPO plan?  What state are you in?  I'll start with that and then see where this goes!
