Permission form

I am looking for a form that I can have my client sign which will give me permission to consult with their physician &/or psychologist.  This has been more difficult to find than  I thought it would be.  Does anyone have one they're willing to share, or can advise where I can find one?

ABMP has a physician's permission and physician's referral form available for download in the member section at abmp.com. But that might be somewhat different from what you're talking about. Is this a one-time thing with a particular client or a regular thing?

Carrie, Thanks for your reply.  It is different.  I need something for my client to sign that gives me permission to discuss their treatment with others professionals.

Gina,

It sounds like the ABMP client forms are too specific for what you're looking for, which is just a piece of paper your client can sign stating that you can speak with their other health professionals.
