How many of you use intake forms when doing chair massage? Do you use them all the time or only when you have consistently recurring clients, such as in a corporate massage setting? Are they different than the ones you use for table massage?

I use them all the time...(liability is still a factor no matter what kind of massage) except when I do chair massage for my own events at my office.
It is especially helpful to get contact information for people who may not otherwise be exposed to massage. Contact information is great for newsletters, postcard, and other direct mailings. You can even have them check a yes or no box about further contact.

I am still new to all of this. I would be most grateful to see what things/ contraindications, etc. you suggest to put on a release form for chair massage? or do you just make it a general release of liability?

The school where I learned massage just had a basic waver sheet everyone signed.  I however work in a Chiropractors office doing mostly chair massages.  My scheduling program has a full 4 page intake form that everyone gets a link to online.  However for first time walk in clients I use the general wavier sheet.
