Sample waiver for use of tools?

Does anyone out there have an example of a waiver for the use of tools? I use Tbars for Neuromuscular Therapy and while the modality is covered by insurance the use of Tbars is not. Has anyone been in the same situation?

I have seek inversion therapy on an desk, that was much simpler to get in and out of. For which i really took advantage from, But after some time period, i start feeling irritation rather than comfort.
