IL CEU's for the World Massage Conference

I am wondering if anyone has used the CEU's offered by the world massage conference for their requirements, if so did you submit all the ceu's using one out of state form? I'm also wondering which name to use on the form, since they are all signed by Eric Brown and another gentleman, Scott (can't make out his last name)? HElp!
